It's so important to start close and work your way in. So many times I've looked in clear water and not see any fish only to have a one come out of nowhere when a fly swung through. Never assume by visual alone that no fish are there.
Always start short and work your way out. This technique will also help identify and rigging problems before you swing into the meat of the run. Get your fly wet, make sure your leader is laying out straight and take those few moments while short swinging to examine and learn the run's currents. While swinging short take time to read the water. Lots of times I will identify and set up on the sweet spot before I reach it. This gives you the angler a huge advantage in setting up a quality presentation.
